Subscriber Benefits Access

Delta Dental of Missouri has made some changes to our website . These changes will make it easier for you to keep your registered email address current in your Profile. If you have re-registered after 5/13/2013, your registration is current. You should be able to log on as usual. If you have not re-registered please do so now. We apologize for this inconvenience. You will have the option to change your profile information, including your email address, username and password by selecting the Profile option after registering.

In the Subscriber section you can view benefits, view claim history, view your summary plan description, request an ID card, print a temporary ID card and access a printable claim form.

This section of the site is password-protected to assure the confidentiality of your personal healthcare information. All family members over the age of 18 covered by Delta Dental of Missouri need to register in order to access our subscriber applications.
